Moving Locations or Transitioning to a Care Facility

We ensure convenient and timely access to in-home care.  Perfect for seniors who wish to ease their transition home after a stay in hospital or other health-care facility.  We’ll help you get back on your feet and back to your life.

Moving to a new location can be stressful, confusing and disruptive.  Our team of planners and home organizers can make this task far more tolerable and manageable.  Where appropriate, we can arrange to take the entire Home Downsizing burden off your shoulders.  This can be especially helpful to out-of-town family members.

Home Maintenance

Remaining at home as long as possible does not have to be out of reach.  We can help with the general safety, comfort and maintenance of your home and property.  Let us help you install hand rails and other safety and support devices in your home, arrange for contractors, repair persons and service calls.   Our service team is ready to help you achieve your goal of remaining independent at home!

Discharge Coordination

Second Family Staff can provide assistance with mobility, including assisting the Client to walk with or without mobility aids, assisting the hospital staff with transfers into and from wheelchairs and assisting with turning in bed. PSWs ensure Clients are safely moving in bed and can assist with light exercises and ROM.

24/7 Care and Every Day Concierge

Second Family Care provides services every day of the year, including weekends, holidays, after-hours and overnight care along with daytime and overnight respite or palliative care.We are prepared to help you with everything from the every day to the extraordinary.
